Preparing for Your Weight Loss Procedure
Before your Mexico bariatric surgery procedure, Dr. Alberto Aceves will have extensive discussions with you regarding your procedure. You will meet with the nutritionist, internal medicine doctor and anesthesiologist before surgery. They will discuss with you what will happen during and after the operation. Then you will undergo a number of tests necessary to evaluate your health prior to surgery.
Admission and Operation
You may be admitted to the hospital the day before surgery or the morning of your operation. The procedure is performed under general anesthesia and, as explained earlier, is usually performed via laparoscopy.

GASTRIC SLEEVE
The vertical sleeve gastrectomy (VSG) is a new procedure that induces weight loss by restricting food intake. Mexicali sleeve gastrectomy patients actually have 80–85 percent of the stomach removed laparoscopically so that the stomach takes the shape of a tube or “sleeve.”
The gastric sleeve is done lapasorcopically. Surgery is typically under an hour long. After your procedure is finished you will be at the recovery room for approx 2 hours were recovery room specialized personnel will monitor your recovery and make sure you are ready to go back to your hospital room. Your family will be notified surgery has been completed and you are recovering.
GASTRIC BYPASS (RNY)
In this procedure, the stomach is divided into two parts. A small, 1 to 2 ounce stomach pouch is created and produces a significant restriction on the amount of food you can eat at one time. This is the main cause of weight loss. The remaining stomach is completely stapled shut and separated from the small pouch, but not removed. The small intestine is then divided with the lower portion brought up to connect directly into the newly formed small pouch. This provides an outlet to the pouch for which food can travel. The connection of intestine to pouch is made purposely small. This small outlet will hold the food in the pouch for longer periods of time thus reducing hunger.
DUODENAL SWITCH
This part of the process is virtually considered a Vertical Sleeve Gastrectomy*. Patients that have been through Duodenal Switch will experience food restriction rates that will change over time within a process called ‘pouch maturation’, reached about 9 to 12 months after DS surgery. At first stance, the patient’s stomach seems small, and it’s the starting point for weight loss. Then the stomach will stretch out gradually until reaching a food intake about half or two-thirds the amount of food they would eat before undergoing a Duodenal Switch procedure. And even though the stomach becomes of a smaller size after DS, none of its functions change.
LAP BAND
The band component of the Lap Band System is fastened around the upper part of the stomach to create a small stomach pouch. Part of the lower stomach is sutured over the band. The rest of the lower part of the stomach remains in its normal position. The reservoir, used for adjusting the band, is placed through a small incision. To accomplish this, the surgeon usually slightly enlarges one of the incisions which have already been made for one of the laparoscopic tubes. Dr. Aceves has helped more than 2500 Lap Band Mexicali patients lose weight.
REVISION SURGERIES WE PERFORM:
Band to Sleeve Surgery, Band to Bypass Surgery, Sleeve to Sleeve Surgery, Sleeve to Bypass Surgery and Sleeve to DS Surgery.
Patients may inquire about revision weight loss surgery for several reasons. A single weight loss surgery procedure is sufficient for many patients, providing adequate long term weight loss. For others, a single weight loss surgery procedure may not cause the desired results, either through insufficient weight loss, poor resolution of co-morbidities and/or due to medical complications resulting from the weight loss surgery itself.
OPEN SURGERY
Sometimes laparoscopic surgery is not an option. This may be the case if you have had a previous abdominal operation and adhesions are present (i.e. areas of tissue have grown together). Or, during the laparoscopic operation, the surgeon may decide to switch to the “open” method for a number of reasons, such as bleeding, visibility, etc . A larger incision will be made in the abdomen (so that the operation can still be performed), but you will be unaware of this while you are under the anesthesia. After this open operation, you will probably stay in the hospital longer as there could be more complications, and it may take more time to return to your normal activities. Dr. Aceves has never had to turn into and open operation with a weight loss procedure but there is a chance that it may happen in a special circumstance
What to Expect After Weight Loss Surgery
Once the anesthesia has worn off, you may be in some pain for a while but you will not be in agony, mediciton is given to relieve the pain. We provide scheduled pain medication for patients while at the hospital to ensure a comfortable recovery to the extent posible. It is very important for you to get out of bed and start moving as soon as possible in order to prevent blood clots, respiratory problems and bedsores. We will also ask you to start doing breathing exercises at the hospital. Our nurses will encourage you to follow the doctro’s instructions to make your recovery faster and uneventful.
For the Gastric Sleeve Surgery (VSG):
Your typical hospital stay will be 3 days. You have your first leak test the day of surgery in the OR. You will have an X ray with a contrast solution , your 2nd leak tests, the day after surgery to check for leaks and to see your new sleeve. Two days after surgery you will have your 3rd and final leak test, this consists of drinking apple juice with a blue dye and checking your drainage. After this tests you start your liquid diet. In the absence of complications (and depending on your occupation), you should be able to resume your normal activities within a week or two following the laparoscopic procedure.
For the Lap Band (Laparoscopic Banding):
An X-ray is usually taken one day after the operation to ensure that the LAP-BAND System is in the right place and that the new stomach outlet is open. You may be asked to swallow a liquid which can be seen on X-ray (barium) . Typically, after the laparoscopic procedure, your hospital stay will be between 1 and 3 days. The hospital stay may be longer following the open procedure. In the absence of complications (and depending on your occupation), you should be able to resume your normal activities within a week or two following the laparoscopic procedure.
For the Gastric Bypass ( RNY) and Duodenal Switch Surgery (DS):
Your typical hospital stay will be 4 days. You have your first leak test the day of surgery in the OR. You will have an X ray with a hydro soluble contrast solution the day after surgery to check for leaks ( quit algo aquí) . 2 days after surgery you will have your 3rd and final leak test, this consists of drinking apple juice with a blue dye and checking your drainage. After this tests you start your liquid diet and remain in the hospital for 2 more days recovering. In the absence of complications (and depending on your occupation), you should be able to resume your normal activities within a week or two following the laparoscopic procedure.
POST OP DIET
For all weight loss procedures patients need to follow post op healing diet. This diet is not intended for weight loss although you will lose weight rapidly in the healing diet. This diet is to help your stomach heal and prevent any complciations. The diet varies a little depending on the procedure you had done. We will provide written instructions and you will have ample opportunity to ask any question to our doctors and nutritionist when you are here for surgery and recovery. The diet is divide into 3 sections:
- You will start by following a clear liquid diet for 10 days after surgery, your stomach is healing and is not ready to receive anything heavier than clear liquids. You will be given goals for liquids, calorie and protein intake during this stage. Getting enough liquids is of outmost importance especially in this stage.
- The 2nd stage consists on full liquids for 10 days . You will be able to eat a little bit heavier liquids, your stomach will be ready to receive them at this stage. You will start with your vitamin and supplements at this stage as well.
- The 3rd stage consist on soft foods for 10 days . We will provide examples of what soft foods consist of to help you plan your meals. As in the other stages we will provide goals for protein, calories and water daily intake. It is on this stage that you will start eating more in a more structured way, similar to what your eating life will be with your surgery.
- One month after surgery you may start with solid food if and only if you have felt comfortable with each of the previous stages.
